Summary: | The aircraft crashed shortly after takeoff while attempting to make an emergency landing after the pilot reported problems with the left engine. Vibration caused nuts to work loose from the engine turbine which resulted in engine failure. No fault was found with the right engine and it is believed that it may have been inadvertently shut down by the crew. |
